He knows he should be careful.  He has drunk too much, and she has drunk hardly at all. He can tell by the way she holds her drink.  The glass has a stem, but she holds it in a way that would warm the ice.  She has to hold it that way of course, if she is going to lean that way on the bar with her elbow.  It shows she would rather be patient and wait than enjoy her drink.
 
 Such women have always been the most dangerous, because they know how to break a man's heart.
